Mitral Regurgitation Index –

Grading of 6 variables

Points 0-3 for each

Total points divided by 6 is the MR index

< 1.7 is mild MR

2.1 is severe MR

Jet Penetration –

O – No jet

1 – Central jet not impinging on lateral wall in any view

2 – Eccentric jet upto first pulmonary vein

3 - Eccentric jet encircling LA beyond pulmonary vein

PISA –

0 – No PISA

1 – <= 0.5 cm

2 – 0.5-1 cm

3 - >= 1 cm

CW Jet intensity and character –

0 – No jet

1 – Incomplete jet envelope

2 – Complete jet envelope, jet density 20-50% of inflow

3 – Complete jet envelope, jet density 50-70% of inflow

PA Pressures –

0 – PAP < 25 mmHg

1 – PAP  25-30 mmHg

2 – PAP < 31-45 mmHg

3 – PAP > 45 mmHg

Pulmonary vein flow pattern -

0 – Systolic dominance, systolic impulse exceeds diastolic impulse by 50% or more

1 – Systolic dominance, systolic impulse exceeds diastolic impulse by < 50%

2 – Diastolic dominance

3 – Systolic flow reversal

LA size -

0 – Normal

1 – Mild enlargement

2 – Mild- moderate to moderate enlargement

3 – Moderate-severe to severe enlargement
